Private Training Registration

I hereby forever release and discharge Best Paw Forward, Valerie Masi, employees or any person acting on its behalf, from any claims, liability action for damages, compensation or otherwise known, on account of or arising out of dog training classes, socialization classes, personal training classes, and/or your pets' or demo dogs' behavior. You take full responsibility for your pets' behavior during training and thereafter.

I release and discharge all liability from all companies, agencies, officers and persons providing good faith, pertinent information and/or records as requested to successfully complete your pet's training.

All classes are non-refundable and training is not guaranteed.

I understand Best Paw Forward Dog Training requires 24 hour notice for all cancellations or I will be charged or lose the session.
